Instructions

Fill out the following form, and click on Submit.
At a minimum, we will need the info on the following survey to start your web site.

	Name of your company, and contact information (phone numbers, fax, email).
	Colors that you want to use for the background and text.
	Type of site and information about your business.
	Once you have filled out the following form, email us pictures that you would like to use on your web site by clicking here and attaching the picture.
	The site will be posted, you can review it, and let us know what you would like changed.

Type of Site Desired:
Keywords (Will be used when your site is submitted to search engines)	Enter here the specific keywords that you think people would use to search for your site on the search engines
If you want a product catalog or ecommerce site, do you have a list of the items available in a:	Database Excel Spreadsheet Word Document (We will email you asking for the information you have.)
Email Addresses List the names you would like set up as email addresses for your domain, such as Sales@yourdomain.com.

Agreement Do not change.	I agree to pay Taylor Business Software the monthly hosting fee for hosting the website, the cost of developing the website, and, if required, the annual domain name registration fees.
